Mine is secured with bolts, so yeah, they had to drill. this reminds me, I need to get it repainted. There are 3 mounting points for the Pajero Sport/Nativa style spoiler. Actually if you look at the lift gate where the spoiler would bolt to you will see factory indentations where you would drill for the wing. Very easy to install even without a template.

Inside or outside? No external indentations on mine. Internal I see pinchwelds near where hinges are.

I had that spoiler factory on mine and I absolutely hated it. I just brought mine to the yard. I removed mine and stuck plugs into the old holes (after painting them white).

I contacted one of the guys comes to the dealership putting on the spoilers Says he's going to get me one painted and put on for a buck and a quarter

I just installed the black rear deflector. I picked up a factory one from the junkyard a couple of months ago and finally got around to putting it on. I marked the locations for the holes, drilled, installed nut rivets, and screwed it on. It turned out great! (I don't know if you can see the picture or not but its linked over from ExpeditionPortal.com)

Can't see the pics....haven't got the chance to install mine, what size hole did you bore & did you treat with paint or something to prevent rusting?

Try this picture: On the hole, it depends on the size of your insert. I tested mine on some scrap metal fist to figure out the right size. I didn't use paint but just a small coating of RTV around the insert should keep everything protected.
